American Medical Holdings Inc. was established in 2009 in Staten Island, NY, by Vladimir Badmaev, MD, PhD, to develop nutraceuticals and natural pharma products at www.badmaevtradition.com. His lifelong goal and work are to integrate traditional botanical knowledge with science-based pharmacology, which he describes as the Interactive Nutrients Process (INP), to design safe and effective “smart drugs” (Badmaev, V. 2024).

The medicine of tomorrow may depend on integrating traditional medical systems such as Ayurveda, Tibetan, and Chinese medicine into contemporary medical practice. Therefore, the author has proposed integrating traditional botanical knowledge with science-based pharmacology, described as the Interactive Nutrients Process. Knowledge of botanical treatments may enrich an often one-sided, symptom-oriented practice of contemporary pharmacology, yielding novel treatments rooted in generations-old health care wisdom.

The dynamically evolving science of pharmacology requires AI technology to advance a new path for drug development. The author proposes using generative AI to identify suitable drug molecules for future medicines, building on the wisdom, experience, and intuition of traditional materia medica and the respective traditional medicine practitioners. The recently described principles of new drug development (Badmaev, V. 2024) are rooted in the tradition and practice of Tibetan medicine. An illustrative example of the outcome is a potential small-molecule drug, 6-Shogaol and related shogaol derivatives, from ginger roots (Zingiber officinalis, fam. Zingiberaceae), evaluated in two open-label clinical studies in patients with the precancerous hematological condition myelodysplastic syndromes (MDS).

The two clinical studies (2017 and 2025) evaluated 6-Shogaol in a daily dose of 20 mg for up to 12 months, examining biological markers of iron homeostasis and hematopoiesis and preventing cytopenias in patients with early-stage myelodysplastic syndromes. 

In a 2017 study (Golombick, T.), 6-shogaol supplementation in patients with early-stage MDS may selectively “smart drug” regulate serum ferritin levels and the iron-regulating hormone hepcidin, thereby decreasing iron overload and improving hematological parameters, alleviating cytopenias, enhancing quality of life, and preventing comorbid conditions, e.g., liver function deterioration. 

The 2025 study confirms improvements in cytopenias among patients with low-grade MDS and indicates novel therapeutic potential for 6-Shogaol therapy. It is well recognized that in patients with MDS, improvement in cytopenias is not expected without the administration of growth factor supports such as granulocyte colony-stimulating factor (G-CSF) or erythropoietin (EPO). These therapies can also be associated with adverse events. In the 2025 study cohort, none of the participants with improved cytopenias required these growth factor supports. Furthermore, as they were not required, there was no significant financial burden on the patient for the use of growth factors GCSF or EPO. Hence, 6-shogaol therapy offers a safe alternative for patients with low-grade MDS to improve cytopenia.

Additionally, in patients with CMML (chronic myelomonocytic leukemia), monocytosis improved, which is not usually expected without chemotherapy. Six of the eight CMML participants (75%) showed improvement in monocytosis (a condition indicating increased proliferation of abnormal blood cells that can lead to AML) with 6-Shogaol therapy after 24 weeks of treatment. After ceasing therapy, five out of the six participants developed deterioration of monocytosis, returning to baseline. Therefore, 6-Shogaol may offer novel therapeutic benefits in improving monocytosis in CMML patients.

In summary, 6-Shogaol was well tolerated, with clinical and laboratory benefits observed in patients with low-grade MDS. Approximately 50% of patients showed improvement in their cytopenias in this study. Improvement in cytopenias is usually seen with growth factor therapies, which come with significant side effects. We are not aware of any conventional or alternative therapies that can improve cytopenias in patients with low-grade MDS.
